Skilled Nursing

The skill and knowledge of our nursing staff enables them to coordinate care, oversee patients' progress, and assist patients/caregivers to understand:

• Stages of illness and recovery

• Safe administration of medication

• Diabetic Care/Supplies/Teachings

• Wound Care/Supplies

• Recognition of symptoms in recurring illness

Rehabilitation Services

• Physical Therapy

• Occupational Therapy

• Speech Therapy

Home Health Aide

Under the direction of case manager or medical staff:

• Ambulation Assistance

• Assisting with simple prescribed exercises

• Check vital signs (Blood pressure, pulse rate)

• Document the patient's condition and progress

• Provide personal care (Bathing, dressing, and grooming)

Medical Social Worker

A medical social worker helps patients and members of the family with social and emotional difficulties which may result from illness.

A medical social worker helps patients and caregivers with community resources such as meal delivery, transportation, and other services.

 

Medical Equipment

The Agency can assist patient in securing medically necessary equipment such as wheelchair, walker, hospital bed, or any other assistive devices as prescribed by the physician.
